A human cannonball performing at a circus in Chile overshot his safety net and crashed into the crowd.  According to one report, he hit a clown

who was knocked to the ground, but not hurt.

The cannonball did guy survive, but was busted up pretty badly with a fractured chin and knees.  The guy who operated the cannon took the blame for firing him into the air with too much pressure.
